Si uno o ambos padres que están vivos no asisten al viaje, el menor deberá descargar el Formulario G, completarlo, hacerlo notarizar (notario), escanearlo y tenerlo listo para subirlo para completar el formulario individual del menor. El menor debe guardar el Formulario G con su pasaporte durante todo el viaje.
The minor should keep the hard copy of Form G with their passport for the duration of the trip.
